We offer health programs that address all aspects of your being.
Our multi-disciplinary programs include the core components of fitness, nutrition, mind/body connection, and education. Tailored to your specific health needs, they provide innovative treatment, practitioner collaboration, informed knowledge, and compassionate support.
Each of our programs at Connect Health are specially designed to build upon each other; beginning with the first stage of gaining a clear awareness of your current situation, leading to targeted strategies for lasting change, and resulting in ongoing connection, knowledge, motivation and support.
To participate in a comprehensive health program:
Step 1: Attend a free Q & A session.
Facilitated by one of our integrative physicians, this casual one hour session will give you a quick overview of integrative medicine, a description of our Connect Health program offerings, and a chance to ask any questions you may have. Learn more.
Step 2: Sign up for our Foundation Program.
The Foundation Program is an experiential workshop delivered by our integrative physicians and practitioners. This engaging and enlightening workshop is designed to educate participants on the benefits of integrative medicine, including practical tools and up-to-date information on key components such as stress reduction, illness prevention, chronic disease management, and nutrition.
Step 3: Book an integrative consultation.
This comprehensive visit with one of our integrative physicians consists of a careful review of your health history, a physical examination, and the discussion of further diagnostic tests, if required. Your physician will take the time to get to know you, clarify your main areas of concern, and introduce you to a variety of beneficial therapies and approaches. Together, you will identify other members of our multi-disciplinary team you may wish to collaborate with, beginning to formulate a strategy of care.
Step 4: Access health planner support.
The heath planner is a supportive liaison who will help you discover and clarify your intention, and encourage you in implementing a health plan. Our health planners are registered nurses and nutritionists whose job is to help guide and coordinate your care plan, providing emotional support, access to resources, and pertinent information as required. They are a valued advocate for your care and your main point of contact you can call on to answer questions, help navigate the health care process, and communicate with your care team, if necessary.
Step 5: Receive core practitioner assessments.
A key component to integrative care includes evaluation and treatment in the areas of nutrition, movement and fitness, and mind/body therapies. Our comprehensive evaluation program (Begin) includes assessments from practitioners trained in these modalities. Following these assessments, they will contribute their recommendations to your care plan, collaborating with each other, and your integrative physician, if necessary.
Step 6: Participate in educational workshops.
Many of our programs include access to a variety of workshops at our centre on topics such as, mindfulness, stress reduction, healthy aging, women’s health, nutrition tips, and more. These workshops are an important aspect to the integrative care model, and are designed to give you the tools and skills you need to take charge of your health.
The following is a summary of our integrative health programs:
- Begin
A comprehensive multi-disciplinary evaluation program to assess your current health situation, and develop a personalized plan for moving forward.
- Transform
An intensive 8 or 12 week program designed for those who want to make a dramatic shift in their health and lifestyle within a short period of time.
- Prevent
A one year program consisting of ongoing care and support from your integrative physician and health planner in order to discover integrative treatment solutions, maintain desired health, and take steps to prevent illness.
