Engage+ is a comprehensive evaluation program.
Through in-depth consultations with members of our multi-disciplinary health care team, this program will determine where you are now, what your health risks are, early detection strategies, integrative treatment solutions, and ways to prevent illness and disease. The result is a thorough assessment of your health situation, and a clear and personalized plan for moving forward.
This program asks the following key questions in order to develop the best plan for you:
- How do you eat?
- How do you move your body (exercise)?
- How do you breathe?
- How do you use your mind?
- How do you respond to stress?
Program Components
Pre-assessment:
The first step in the evaluation process is the completion of a comprehensive intake form. This form includes your medical and family history, as well as relevant lifestyle, fitness and nutritional information, providing a well-rounded view of your current health situation. This pertinent information will help your integrative physician properly assess symptoms, root causes and potential health risks.
In-depth intake by an integrative physician:
This comprehensive visit with one of our integrative physicians consists of a careful review of your health history, a lifestyle assessment (including stress, nutrition, and fitness), risk assessment, physical examination, and the discussion of further screening and diagnostic tests, if required. During this intake, your physician will take the time to get to know you, clarify your main areas of concern, and introduce you to a variety of beneficial therapies and approaches. Together, you will identify other members of our multi-disciplinary team you may wish to work with, beginning to formulate a strategy of care.
Nutritional assessment:
You will meet with one of our registered dietitians or nutritionists who will work with you to assess your nutritional needs, determine habits and patterns, and discover personalized solutions to fit your life. They will develop an individualized plan with you, which includes recipes, meal planning, educational materials, resources, and support.
Movement and fitness assessment:
This component includes a detailed evaluation by a certified fitness professional. They will work with you to determine your current fitness levels (including cardiovascular, postural, muscular, and functional), set achievable goals, and develop a personalized fitness plan to fit your lifestyle and health situation.
Mind/Body consultation:
The relationship between the mind and body is a powerful one, each having a profound and sometimes lasting effect on the other. Our mind/body component honours the power of this relationship, and its effect on your overall health. Our practitioners will help you determine your needs, and work with you to discover techniques to alleviate stress, gain awareness, and transform unhealthy patterns into those that are healthy and nourishing.
Health Planner support:
A Health Planner is a supportive liaison who will encourage you in developing a personalized health plan. Our Health Planners are registered nurses and nutritionists who will guide and coordinate your care plan, providing emotional support, access to resources, and pertinent information as required. They are a valued advocate for your care and your main point of contact you can call on to answer questions, navigate the process, and communicate with your care team, if necessary.
Personalized Health Plan:
A Personalized Health Plan is a written care plan integrating physician recommendations and assessments with those of our nutrition, fitness and mind/body practitioners. The health plan addresses all aspects integral to health and wellness, and outlines practical strategies and integrative treatment solutions specific to your situation. It also includes valuable resource materials and handouts relevant to your needs. The result is a comprehensive and personalized care document outlining the best strategies for optimizing health.
The Begin Program will result in:
